I. Liability Release, Parental Consent

In consideration of the acceptance of my application for the above program/classes, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages of which may hereafter occur to me, or the minor children for which I am legally responsable, as a result of participation in said event. This release is intended to discharge in advance Abrahao Brazilian Jiu Jitsu LLC, Blue Zone Surf & Fitness, its officials, officers, employees, volunteers, agents and/or hired services from liability without exception.

I understand that the included activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on my heirs and assigners.

I, {name}, plan to participate in some or all of the activities included in the above programs/classes, and I execute the above liability release on my own behalf.

Parental Consent (Complete for applicants under 18 years of age)

I agree that I am the legal parent or guardian to the minor listed on this registration. I give consent for my children who are listed to participate in the activities included in the referred to programs/classes, and I execute the above liability release on their behalf.

II. Consent for Treatment

I hereby give consent to have the above applicants treated by emergency medical personnel, a physician, or surgeon, in case of sudden illness or injury while participating in the above activity. It is understood that Abrahao Brazilian Jiu Jitsu LLC,  Blue Zone Surf & Fitness will provide no medical insurance for such treatment, and that the cost thereof will be at my expense. I have read and understand the foregoing liability release and parental consent form, and agree to all of its terms and conditions.

III. Photo Release Form for Adults and for Minor Children

I hereby authorize Abrahao Brazilian Jiu Jitsu and Blue Zone Surf & Fitness to publish the photographs taken of me and/or the undersigned minor children, and our names, for use as promotional material, including, but not limited to websites, ads, social media, flyers and for display in the facility. I release Abrahao Jiu Jitsu and Blue Zone Surf & Fitness from any expectation of confidentiality for the undersigned minor children and myself and I attest that I am the parent or legal guardian of the children listed and that I have the authority to authorize this use of photographs, films and names. I acknowledge that participation in publications and websites produced by Abrahao Brazilian Jiu Jitsu and Blue Zone Surf & Fitness is voluntary, neither the minor children nor I will receive financial compensation. I further agree that participation in any publication and website produced by Abrahao Brazilian Jiu Jitsu LLC and Blue Zone Surf & Fitness confers no rights of ownership whatsoever. I release Abrahao Brazilian Jiu Jitsu LLC and Blue Zone Surf & Fitness, its contractors and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.

IV. Participation & Payment agreement

Registration for participation in classes at Tamarindo Jiu Jitsu Academy is a term commitment. Terms run between 8 - 18 weeks depending on time of year. All months included in each term of enrollment require a full monthly tuition payment regardless of attendance, holidays or missed classes for any reason. All students will have access to the full term schedule and important dates by request via email and/or the posted notices at the studio.
